Issues with Your Current Farmingdale Public Library Card Form
Every person living in the Farmingdale School District is entitled to a free library card. Simply provide us with 2 forms of ID that include your name and current address.
Examples of acceptable forms of ID for proof of residency are: car insurance, car registration, checkbook with name and address, license, and utility bill.
Children not in Kindergarten will need the signature of a parent/guardian and a birth certificate.
Children in Gr. K-12 will need the signature of a parent/guardian.

How do I obtain/renew a library card?
You may obtain a card if you live in the UFSD #22 Farmingdale School District.
The library now offers two ways to apply for a card. You can apply in-person at the library, or you can fill out our online application. Library cards will be immediately available with in-person applications. For online applications you have the option to pick up your new card at the library or to have it mailed out to your home within two business days.
New Card Online Application:
To apply for a library card online, fill out the library’s online application form and supply two forms of valid ID. Examples of acceptable forms of ID for proof of residency are: car insurance, car registration, checkbook with name and address, license, and utility bill.

Renew Your Library Card:
Resident library cards expire every three years. You can renew your card in-person at the library or online.
